The Team-based Care Manager is the manager of a team of clinical staff, led by cardiologists, that delivers exceptional care for patients in an innovative model of outpatient care. The care team may include Nurses, Cardiac Physiologists or other clinical professionals depending on the clinic’s purpose. The Team-based Care Manager will work closely with doctors and other team members to ensure the team achieves the best possible outcomes for patients through teamwork, innovative practice, and optimal use of technology.
The key responsibilities for this role are:
Ensuring that the care team is appropriately resourced with the number and type of staff need to meet clinical and operational requirements, including working as a member of the care team as required
Day to day line management of a team of Cardiac Nurses
Planning and rostering of non-Nurse team members in consultation with other managers
Ensuring the team performs to the required standard of clinical care and follows GenesisCare policies and procedures, including the use of digital tools and data capture systems to support patient care and organisational priorities
What you’ll do:
Contribute to local practice leadership and implement national strategy:
As a leader, work closely with the Operations Manager and other members of the practice leadership team to ensure delivery of the practice objectives
Work closely with national leadership team members to align the operations and performance of team-based clinics with the organisation’s strategy and implement changes as the innovate model of care evolves
Manage the care team to deliver new model clinics:
In consultation with Operations Manager, assess workload and skills required in the care team and undertake recruitment and onboarding activities of the care team members as needed, including from internal teams
Plan and roster team members (including self as needed) to ensure the team is ready to operate clinics as agreed by Operations Manager
Review and manage the ongoing performance of the care team according to agreed operational and quality requirements
Work with national managers to ensure that the care team is operating according to strategic and operational requirements
Monitor overall clinic utilization and financial performance and work with practice management to optimise
Manage a team of Cardiac Nurses
Recruit, train and undertake day to day management of Cardiac Nurses to ensure there are sufficient Nurses of appropriate skills to work in the care team
Work closely with national managers to ensure competencies are maintained at the required level, including continuing professional development is maintained
Manage performance of the Cardiac Nurse team according to the Cardiac Nurse
Other duties relating to the delivery of patient care or research may be required from time from time.
Who you are:
At least 3 years experience in a management role including leadership of clinical teams
At least 5 years experience as a nurse, with at least 2 years experience in cardiology.
Must have undergraduate nursing degree
Current AHPRA registration as a nurse is essential
Proven skills in managing the delivery of patient care through teams
Proven ability to work successfully as a member of a leadership team
Experience in a busy, time pressured clinic environment
Familiarity with the use of IT systems both to support clinic operations and data management
Travel between sites within a region may be required
