Salem Health Medical Group is currently seeking two providers to see outpatient Polk county patients at our School Based Health Clinic and Independence Primary Care Clinic. These clinics are located in the beautiful, rural community of Independence, Oregon.
Independence Family Medicine Clinic, is seeking a full time provider to join our established team, including 1 Family Medicine Physician, and 1 Physician Assistant. The NP or PA hired into this position will assume responsibility for a full patient panel. Salem Health Medical Clinic employs the only primary care providers in the community of approximately 10,000 residents.
Our certified School-Based Health Center, is seeking an experienced Family Nurse Practitioner or Physician Assistant to provide care to Central School District students and the surrounding communities. This unique role allows the APP the opportunity to participate in a team approach to care with other community partners, including dental and behavioral health providers who also operate their practices within the health center.
The Monmouth Family Medicine Clinic, located in the beautiful, rural community of Monmouth, Oregon, is seeking a Family Nurse Practitioner or Physician Assistant to join our established team, which includes 4 full-time physicians, and 1 full-time physician assistant.
