Your Role With Us:

Salem Health Medical Group is currently seeking two providers to see outpatient Polk county patients at our School Based Health Clinic and Independence Primary Care Clinic. These clinics are located in the beautiful, rural community of Independence, Oregon.

Independence Family Medicine Clinic, is seeking a full time provider to join our established team, including 1 Family Medicine Physician, and 1 Physician Assistant. The NP or PA hired into this position will assume responsibility for a full patient panel. Salem Health Medical Clinic employs the only primary care providers in the community of approximately 10,000 residents.

Our certified School-Based Health Center, is seeking an experienced Family Nurse Practitioner or Physician Assistant to provide care to Central School District students and the surrounding communities. This unique role allows the APP the opportunity to participate in a team approach to care with other community partners, including dental and behavioral health providers who also operate their practices within the health center.

The Monmouth Family Medicine Clinic, located in the beautiful, rural community of Monmouth, Oregon, is seeking a Family Nurse Practitioner or Physician Assistant to join our established team, which includes 4 full-time physicians, and 1 full-time physician assistant.

Position details:

• Work Schedule: Monday-Friday, day off during the week
• Base salary upon hire, productivity-based compensation after 12-months of hire
• Annual group incentive bonus program ~ up to 20% base compensation
• Relocation assistance & start bonus
• $4,000 + 40 hours annual CME allowance
• 200 hours PTO
• Up to $30,000 education assistance

You may be a qualified PA candidate if you:

• Have graduated from a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or its predecessors, e.g., CAHEA, CAAHEP.
• Have passed the Physician Assistant National Certifying Examination (PANCE) and are certified by the National Commission on Certification of Physician Assistants (NCCPA), or can complete the examination and certification requirement within in 12-months of initial appointment to the Medical Staff.
• Current PA license in good standing issued by the Oregon Medical Board (OMB).
• Hold DEA certification.
• Maintenance of American Heart Association BLS certification.
• Spanish speaking required for our Independence Clinic, preferred for School Based Health Clinic

You may be a qualified NP candidate if you:

• Possess a Master’s Degree or higher in Nursing, or in a field deemed to be equivalent by the Oregon Board of Nursing.
• Have satisfactorily completed a Nurse Practitioner Program recognized by the Oregon Board of Nursing.
• Hold a current, unencumbered Registered Nurse license in the State of Oregon.
• Hold a current, unencumbered Certificate as a Nurse Practitioner through the Oregon State Board of Nursing.
• Must be able to demonstrate experience in the specialty area in the past 24 months. If this requirement is not met, an onboarding plan may be developed for review and consideration.
• Hold DEA certification.
• Hold current American Heart Association BLS certification.
• Spanish speaking required for our Independence Clinic, preferred for School Based Health Clinic
