Job title: Band 6 Occupational Therapist Salary: £26,302 - £35,225 per hour depending on experience Length: Permanent Location: EssexAre you an enthusiastic and passionate individual who is looking to expand and grow your career in Occupational Therapy? An excellent permanent opportunity has arisen in the Essex area for a Band 6 Occupational Therapist working for a well established NHS trust.This position is to work as a specialist Occupational Therapist responsible for providing a high standard of assessment, advice, treatment and evaluation of patients referred to the Frailty Assessment Service. This may include patients with diverse presentations and complex physical and psychological conditions, including dementia.The Occupational Therapist will also provide integrated and patient centred care, working as part of a multi- disciplinary team in the Frailty Assessment Service, to enable patients with urgent care needs to remain at home where possible, and linking to primary and community care to achieve this.

Requirements

  • *HCPC Registered
  • *Degree in Occupational Therapy or equivalent
  • *To have completed a wide range of clinical placement and experience during training and junior posts
  • *Experience of working with frail and older people
  • *Experience of working within a multi disciplinary team

Responsibilities

  • *To undertake and contribute to the completion of the Comprehensive Geriatric Assessment with patients in the Frailty Assessment Service.
  • *Use well developed clinical reasoning to select appropriate assessment techniques that lead you to determine a clinical diagnosis of the patient’s needs.
  • *To analyse the information gained from the assessment and share with the patient and the mutli –disciplinary team to facilitate identification of problems and make arrangements for a safe discharge.
  • *Discuss and agree goals and outcomes for treatment or management with patient and identify and refer on to community, primary care, or social care services where appropriate.
  • *To ensure the patient understands and agrees to the treatment plan giving consent to it. This will involve negotiation and motivation skills.
  • *To select appropriate treatment techniques/intervention from resources available, the therapists specialist skills drawing on past experience.
  • *To produce clear written information / programmes for patients to follow at home or when not with a Therapist.
  • *To undertake home assessments (Access Visits or Home Visits) according to clinical need.
