Summary
Under the direction of the Asst. Director the Program Navigator assists in the coordination and implementation of long-term services and support in the community. At CCC all duties are performed in a manner that fosters the achievement of the organization’s mission.
Essential Functions- Provides Referral Screening for all new potential clients of Public Programs supported by CCC.
- Provides telephonic communication with client and/or the referral source to provide education and program navigation.
- Schedules initial assessments
- Follows, supports and assists pending client's post-assessment to assist with timely activation onto appropriate program/s. This will include home visits as needed.
- Participates in and works effectively as part of an interdisciplinary team.
- Assist in mentoring new staff as needed.

RequirementsEducation:
High School Diploma required; Associate Degree in Health or Human Service preferred
Experience:
- For High School graduates, at least two years’ experience in a human service setting preferred.
- For attained Associate Degrees, at least one year experience in a human service setting, or equivalent experience preferred.
- Project management and organizational skills.
- Strong verbal and written communication skills.
- Knowledge of community resources preferred.
- Ability to maintain confidentiality of client and company information.
- Ability to understand and articulate multiple programs.
- Working knowledge of Connexus.
Other:
- Reliable transportation, valid driver’s license and current automobile insurance.
