JOB DESCRIPTION Job Description Position Summary The duties of a Medical Scribe are to document the dictated patient history, physical examination, family, social, and patient medical histories as well as document procedures, lab results, dictated radiographic impressions and any other information pertaining to the patient’s encounter in the clinic. The scribe will also perform duties of the Medical Assistant to include measuring and recording vital signs, preparing patients for examinations, cleaning exam rooms, and requesting medical records. Essential Duties and Responsibilities The essential functions include, but are not limited to the following: * Assigned to work with one provider, but may be required to cover for another scribe, as needed. * Accompany the provider upon patient interview and examination. * Document the dictated history, including history of present illness, review of systems, past medical and surgical histories, family and social histories, medications and allergies. * Document physical examination findings and procedures as performed by the provider. * Document the results of laboratory and radiographic studies as dictated by the provider. * Document the correct time of patient care related activities, including provider to provider communication, family communication and re-examination of the patient. * Obtain test results and medical records as requested by the provider. * Measure and record vital signs. *
